What is accidental death Insurance?
Accidental Death Insurance provides a benefit to your beneficiary(ies) in the event you die from a covered accident.

Can I be Denied Accidental Death Coverage?
If you are age 18 or older who has at least 30 full days of continuous subscription service, residing in a state where the benefit is offered and we have your basic insured information on file you are eligible for the accidental death benefit.

Is coverage available for my dependents?
No. Coverage is only available for the primary insured subscriber only.

What Expenses Can Accidental Death Proceeds Cover?
It’s really up to you.  While no amount can bring back a loved one who dies in an accident, proceeds can help reduce the financial burden.  The money can be used towards burial and funeral costs, mortgage, rent, other living expenses, educational expenses, funding retirement, medical costs etc.

What Does Accidental Death Cover and what is the Benefit Amount?
Generally, the plan provides 24 hour accidental death insurance protection resulting in death of the insured against covered accidents anywhere in the world, on or off the job, on business - vacation – at home. We will pay a $100,000 Accidental Death Benefit, if you die from a covered Injury resulting, directly and independently of all other causes, from a Covered Accident while the Policy and Your coverage is in force.  The loss of life must occur within 90 days of the Covered Accident. Please see the Summary of Coverage here for additional information.

What Is Not Covered by Accidental Death?
A few brief examples of injuries or activities not covered by this Accidental Death plan are; suicide, self- inflicted injuries, committing a felony or an assault, participation in a riot or Terrorist Act, bungee jumping, parachuting, skydiving, parasailing, hang-gliding, war, flights (except commercial flights, private aircraft for pleasure flying, passenger in military aircraft flown by AMC), racing speeding contests, not properly licensed as an operator of a motor vehicle, sickness, disease, bodily or mental infirmity, bacterial or viral infection or medical or surgical treatment thereof, intoxication, voluntary ingestion of any narcotic, drug, poison, gas or fumes unless as prescribed by a Physician, active duty service in the military. The Group Master Policy will be the prevailing document for consideration of any payable benefit.

How Do I designate a beneficiary to my policy?

How Does My Beneficiary Receive the Policy Benefits if I am Killed in an Accident?
The beneficiary or guardian of a minor beneficiary must contact the insurance brokerage firm, “American States Group” at (844) 446-7765. A claim must be filed with the insurance company.  The insurance company or insurance agent at time of claim can provide further information.

How do I change a Beneficiary to my policy?
Please click here, enter your state of residence and if the plan is available in your state you will be asked to complete your designated beneficiary form. This information will be important to have on file in the event there is a claim. Should you have additional questions please contact an agent at 1.844.446.7765. 

How do I file an insurance claim? Or what if my beneficiary is a minor (under age 18) at the time of a claim? 
A claim must be filed with the insurance company.  If a claim needs to be filed please contact an agent at 1.844.446.7765. The agent will assist in getting the claims process started with the insurance company. 

When Will My Coverage Begin? 
After the ROK Mobile Group Master Policy becomes effective, and you are eligible for Accidental Death coverage, meaning you are an active subscriber who has at least 30 full days of continuous subscription service residing in an available state.  Additionally, you must complete the subscription process with ROK Mobile by providing the minimum necessary insured information through the ROK Mobile web site.

Once enrolled, when does my Accidental Death coverage end? 
The date you are no longer an active ROK Mobile subscriber for which payment is made. The date the ROK Mobile Accidental Death Master Policy should end.  Whichever comes first. 

Who is the insurance company? 
Life Insurance Company of North America or “LINA” is the insurance company who is underwriting the Accidental Death coverage.  LINA is a member underwriting company of Cigna.

What states is this Accidental Death coverage not available in? 
Alaska, Delaware, Kansas, Maryland, Michigan, New York, Oregon, Texas, Utah,Washington.

If I initially sign up for ROK Mobile Service in an available state then re-locate to an unavailable state, can I still keep my coverage?

If I activated my ROK Mobile subscription through a retail store location am I automatically eligible for this insurance benefit?
No. You must provide your basic insured information (full legal name, mailing address, email, etc.) to ROK Mobile thru the website to complete the subscription process to be eligible for the Accidental Death Insurance benefit.
